Chest liposuction removes fat from the chest and the sides of the chest through very small incisions. For some trans men and non-binary people with a small chest that is made up mostly of fat, it can create a flatter contour without the scars of top surgery. In our practice, liposuction is most often used during top surgery to contour the sides of the chest and help prevent “dog ears”; liposuction on its own is rarely enough for a flat chest. Dr. Hakki Izmirli performs chest liposuction in Istanbul for international patients.
The breast is made of two kinds of tissue: fat and firm glandular tissue. Liposuction can remove fat, but it cannot remove glandular tissue or excess skin. This is the most important point when deciding whether liposuction alone is realistic.
| Situation | Usual recommendation |
|---|---|
| Small chest, mostly fat, firm skin | Liposuction alone may be enough |
| Small chest with glandular tissue under the nipple | keyhole top surgery, often combined with liposuction |
| Small chest with mild skin excess or large areolas | periareolar top surgery |
| Medium or large chest, skin excess or sagging | double incision top surgery with liposuction to refine the sides |
| Leftover fullness after a previous top surgery | Liposuction as part of a revision |
If glandular tissue is left behind, the chest can still look rounded under the nipples after liposuction. Dr. Izmirli assesses this at your consultation and will tell you honestly whether liposuction alone can give the result you want.
Liposuction alone is usually only considered if you have:
In practice, liposuction alone is rarely the right choice for a flat chest. Most patients get a better and more predictable result from top surgery combined with liposuction.
Recovery after liposuction alone is usually shorter than after top surgery. You wear a compression garment to help the skin settle over the new contour and to reduce swelling. Bruising and swelling are normal in the first weeks; the final contour becomes visible over several months. When liposuction is combined with top surgery, recovery follows the top surgery plan; see aftercare.
Like any procedure, chest liposuction carries risks. These include bruising, swelling, fluid collection (seroma), infection, contour irregularities or dents, changes in skin sensation, and loose skin if the skin does not tighten as expected. If glandular tissue remains, further surgery may be needed to achieve a flat chest. Dr. Izmirli discusses your individual risks at your consultation.
The answer depends on how much of your chest is fat, how much is glandular tissue, and how elastic your skin is. Send your photos for a free online consultation, and Dr. Izmirli will tell you whether liposuction alone is realistic or whether a top surgery technique will give a better result.
